Annual Enrollment Moved to December

To: Mobility Puerto Rico employees

Based on the terms of the new Mobility CWA bargaining agreement, several changes are ahead for Annual Enrollment for 2017.

  1. Take action – A new tobacco user surcharge of up to $600 annually may apply
  2. Take action – A new medical contribution credit of up to $600 is available
  3. Your Annual Enrollment date has been moved to December

Tobacco Surcharge

You are considered a “tobacco user” if you have used tobacco products more than once every two weeks, on average, at any time since Jan. 1, 2016. Tobacco products include cigarettes, cigars, pipes, e-cigarettes, vaporizers and smokeless tobacco.

If you are NOT a tobacco user, you must certify during annual enrollment that you are NOT a tobacco user to avoid the up to $600 annual surcharge. If you ARE a tobacco user, and want to avoid the surcharge, you must complete the Quitnet tobacco cessation program by Dec. 1, 2016*. The program may take 6-8 weeks to complete, so you must take action NOW1.

Medical Contribution Credit

If you complete the questions on the Gallup-Healthways Well-Being 5® Health Questionnaire (HQ)*, you will be eligible for up to $100 per person (limited to enrolled employee and spouse) in annual credits ($8.33 monthly per person) toward your medical coverage contributions. Your Health Matters – Be Well must receive your completed HQ on or before Dec. 1, 2016, for you to receive contribution credits in 2017.

In addition, you will be eligible for up to $200 per person (limited to enrolled employee and spouse) in annual credits ($16.67 per month) if you supply your Biometric Screening1, 2 data by Dec. 1, 2016. This data often includes lab results and other information often obtained during an annual physical.

1 If you have a health condition that might cause you to need an alternative method to qualify for this credit, contact Healthways at 888-665-6455. They will work with you (and, if you wish, with your doctor) to find a wellness program with the same reward that is right for you in light of your health status.

2Genetic Information Nondiscrimination Act of 2008 (GINA)

AT&T neither requires nor requests that you provide genetic information or family medical history for any purpose in connection with the medical contribution credit or the tobacco medical contribution surcharge. The Genetic Information Nondiscrimination Act of 2008 (GINA) prohibits employers and health plans from requesting or requiring genetic information of an individual or family member of the individual, except as specifically allowed by this law. To comply with this law, we are asking that you not provide any genetic information when responding to the Health Questionnaire or providing information concerning the tobacco surcharge. “Genetic information,” as defined by GINA, includes an individual’s family medical history, the results of an individual’s or family member’s genetic tests, the fact that an individual or an individual’s family member sought or received genetic services, and genetic information of a fetus carried by an individual or an individual’s family member or an embryo lawfully held by an individual or family member receiving assistive reproductive services.
