Spouse/Domestic Partner Re-Verification paperwork required

Verizon is committed to providing high-quality health care benefits to employees, retirees, and their eligible dependents. In order to maintain that commitment, eligibility verifications are conducted to confirm that plan eligibility guidelines are adhered to. We are currently performing an eligibility verification for spouses and domestic partners enrolled in Verizon coverage. Please follow the instructions below to complete the verification.

To complete your Spouse/Domestic Partner Re-Verification online, go to verizon.com/benefitsconnection. From the home page, go to Action Center > Alerts > Verify/Review My Dependent(s) Status.

The required documentation must be received by April 12, 2024 or your spouse’s/domestic partner’s coverage will be cancelled effective May 9, 2024.

Your Spouse/Domestic Partner Re-Verification status will be updated and viewable on your Dependent Information page. It could take up to 15 days for your documentation to be reviewed.
